Educational Qualification :
Candidates who have passed B.Sc (Nursing) are eligible to apply for NHM AP MLHP Recruitment 2021
Age Limit : Age limit should be less than 35 years and 40 years for SC/ST candidates
Application Fee :
1. The application fee is Rs. 300/-
2. SC, ST/ OBC candidates are exempted from the fee payment.
Salary : After training, the salary is Rs. 25, 000/- per month
Selection Process : merit in the online entrance exam

1. Log on to the NHM AP official portal i.e, http://cfw.ap.nic.in/
2. On the home page, find and click on the link “Recruitment for the post of Mid Level Health Provider on contract basis”
3. A new page will open and hit the link “Notification for the post of Mid Level Health Provider”
4. Read the CFW AP MLHP Recruitment 2021 notification details provided in it
5. If eligible, then hit the “Apply Online” link
6. Fill all the details in the NHM AP MLHP application form like personal, academic, and other information
7. Pay the fee online
8. Upload the documents (if required)
9. Verify the details and submit the NHM AP application form
10. Take a print out of NHM AP MLHP Application form and keep the hard copy for future reference
